Share the Care
INNOVATIONS' Share the Care program is designed specifically for residents of retirement communities. This unique program allows residents to contract for as little or as much basic personal care assistance as needed to live independently. INNOVATIONS Share the Care Program is tailored to individual needs and offers the flexibility of packages or individual services. Upon request for services, the client is scheduled for an on-site evaluation, to assess the client’s needs, review medical history, and develop a care plan for that client. Assessments are performed on a periodic basis thereafter. The care plan is then communicated to the staff and services are initiated. INNOVATIONS' Share the Care program is managed by a Care Manager, who is experienced and trained in home health care. The Building Coordinator or Care Manager's responsibilities include daily scheduling of all assignments and daily supervision of all services provided. The Team Leader is the day-to-day contact with our clients, family and your building management. A Registered Nurse provides operational oversight of the Share the Care program.
