W-2 Forms |

Take Charge of
your Retirement! |
|
All W-2 forms for 2001 went through
a verification process and were subsequently mailed on January
21, 2002. If you did not receive your W-2 (perhaps you have
moved recently and have not notified accounting of your new
address?) please contact Tracey Kerr in the Accounting
Department at (810) 494-0664. She will be happy to print another
W-2 for you. In addition, the beginning of the year is always a
good time to review your current income tax withholdings. If you
would like to make a change to your income tax withholdings,
please call Stephanie in the Human Resource Department (810)
494-0668 so that she can send you an Income Tax Withholding form
(W-2). Or you can print a form from the IRS website and mail it
to us. The web address is

All
INNOVATIONS
staff are required to take a TB test annually if the test is a
skin test, and every four years if the test is a chest x-ray.
However, if you have been exposed to TB, the requirement is
every two years.
TB skin tests can be administered by an
INNOVATIONS
registered nurse at no cost to you. If you need a chest
x-ray, you must arrange one with your own physician and
INNOVATIONS
will reimburse you for
the cost.
This requirement can be waived if you have had a TB skin test
conducted within one year prior to your date of hire with
INNOVATIONS.
The documentation must disclose the date and results of the
test, as well as the name of the facility that administered the
test.

Rules and Procedures - Midwest and Great Lakes
Region
- Candidates must have a valid RN license and work in
Illinois, Indiana, Iowa, Kansas, Michigan, Minnesota,
Missouri, Nebraska, North Dakota, Ohio, South Dakota or
Wisconsin.
- You may nominate as many people as you like, but you may
nominate each candidate only once.
- Nomination forms must be complete (do all THREE steps)
and received by April 30, 2002.

Nominations will be judged by a panel of registered
nurses who will select finalists in each of the eight
categories.
Finalists will be our guests at an awards ceremony August
23 in Chicago, where one winner in each category will be
announced.
Finalists will be notified by NurseWeek and the names
published on our Web site when selections are complete.
Winners and finalists will be profiled in NurseWeek.

- Advancing the Profession: Contributions that have
advanced and strengthened nursing as a profession.
- Clinical Care: Excellence in direct-care delivery in any
clinical setting.

contribution in community-based settings or community
outreach efforts.
- Innovation/Creativity: Innovative and creative
contributions that have improved patient care.
- Leadership: Exceptional leadership of nursing or patient
care services in any setting.
- Mentoring: Providing a positive professional influence,
guidance and support of other nurses in any setting.
- Patient Advocacy: Going beyond the call of duty to
advocate for patients.
- Teaching: Significant contributions in education,
professional development, and/or long term learning of
nursing professionals.

Career Fair |

On February 5,
INNOVATIONS
participated in a career fair
for registered nurses. It was held at Burton Manor in Livonia and
was sponsored by Magic 105.1 and co-sponsored by NurseWeek. There
was a steady stream of candidates that stopped by our display to
express interest in
INNOVATIONS.
We received great feedback about our
reputation in the marketplace and many positive comments about our
nurses and their work. Healthcare professionals, including 58 RNs
visited our booth expressing interest in
INNOVATIONS.
