 Carrier: The Guardian    Carrier: M-Care and Care Choices
  • All regular full-time staff.
  • Group Term Life and Accidental Death & Dismemberment
    • Effective the first of the month after date of hire, or change in status to full-time.
    • Coverage amount equal to one year's salary, with a minimum of $20,000 and a maximum of $50,000.
    • No cost to staff - 100% Employer Paid.
  • Voluntary Supplemental Life Insurance.
    • Eligible the first of the month after date of hire, or change in status to full-time.
    • Coverage is available for staff member in the amounts of $25,000, $50,000, $75,000, or $100,000. Rates are based on staff member's age.
    • Dependant children are eligible for amounts up to 10% of staff member's coverage.
 
  • All regular full-time staff
    • Eligible the first of the month, after ninety days from date of hire, or change in status to full-time.
    • Choice of HMO plans.
    • Employer 80% and Staff 20% contribution for single coverage with card choices.
    • Employer 60% and Staff 40% contribution for single coverage with M-Care.
    • Staff contribution: 100% for dependant coverage.

Voluntary Short-term and Long-term Disability Insurance

 Carrier: Jefferson Pilot Financial Insurance
 
  • All regular full-time Administrative staff.
    • Eligible the first of the month, after ninety days from date of hire, or change in status to full time.
    • Staff contribution: 100% at group rates.
    • Rates are determined by current wage and age.
 
 
 
   
401(k) Retirement Plan  
 Carrier: Comerica Bank    
  • All staff is eligible to enroll during open enrollment periods (January & July) after one year of service and 1,000 hours.
    • Eligible for rollover option upon date of hire. Staff that exercises this rollover option are eligible to participate in 401k contributions immediately.
    • Staff member contribution up to 15% of salary.
    • Discretionary Employer Match contribution up to 50% of the first 6% of staff contribution.
    • Eligible to be fully vested after three years of service of 1,000 hours per year.
 

Voluntary Dental Insurance at Group Premium Rates

  • All regular full-time staff is eligible.
    • Eligible the first of the month, after ninety days from date of hire, or change in status to full-time.
    • Staff contribution: 100% at group rates.
